Aims: Recognizing the critical role of evidence-based policymaking in reducing road traffic mortality, this study investigated long-term spatiotemporal patterns of road traffic-related deaths in Iran from 1981 to 2021.
Materials and Methods: This cross-sectional study used mortality data from the standardized Global Burden of Disease (GBD) database. All deaths attributed to road traffic injuries under the relevant GBD cause-of-death codes were included at national and provincial levels. Analyses were conducted using Microsoft Excel 2026 and ArcGIS 10.8. Age-standardized mortality rates, age- and sex-specific rates, and spatial analyses, including geographic distribution mapping and pattern detection, were performed.
Findings: The age-standardized road traffic mortality rate declined from 73.0 per 100,000 population in 1981 to 19.8 per 100,000 in 2021. Mortality was consistently higher among men than women, and the highest rate occurred among people aged 55 years or older. Sistan and Baluchestan and Zanjan had the highest provincial rates at the beginning of the study period, but both declined substantially by its end. Overall, southern and western provinces had higher mortality rates than central provinces.
Conclusion: The substantial decline in road traffic mortality over four decades likely reflects improvements in road infrastructure, vehicle safety, and traffic regulation. Nevertheless, the persistent burden among younger age groups, sex-related disparities, and stable geographic inequalities demonstrate the need for targeted, province-specific, and youth-focused road-safety policies, including education and initiatives to improve driving culture among young people.
