Objective:
Shift work is implemented in various formats or systems, each of which has its own characteristics, advantages and disadvantages, and can leave adverse effects on various aspects of human life. Decreased life expectancy has an effect on physical and mental health and is observed as the final result of job stress in many shift work jobs including nursing. This study aimed to investigate the factors affecting the life expectancy of shift-working physicians in the emergency department.
Methods:
This is a descriptive-analytical and cross-sectional study conducted in 1402 in Tehran and karaj clinics in which shift-working doctors of the emergency department were included.
The study population consisted of shift doctors in the emergency department of Tehran and Karaj hospitals and clinics with at least 6 months of continuous work experience with fixed and rotating shifts in year 1401.Sampling was done using a stratified method with a sample size of 300 people (95% confidence level).
For this purpose, Miller's Hope Questionnaire along with a questionnaire to collect demographic information was provided to the doctors in different work shifts while explaining the objectives of the study and training instructions and how to complete them. Then the data was analyzed by SPSS software version 26.
Result:
Finally, 313 physicians (158 female) including 94 general physicians (30.0%), 197 emergency specialists (62.9%), and 22 doctors specializing in other fields (7.0%) with an average age of 44.07 ± 7.47 were included in the study. The average score of the life expectancy test in different subgroups of marital status (p<.0001), having children (p<.0001), living with the parent in question (p<.0001), the field of study (p<.0001), working Being a spouse (p=.008), the level of satisfaction with his work situation (p<.0001), the level of satisfaction with his life situation (p<.0001), the desire to change careers in the current situation (p< .0001), travel to get rid of job stress (p<.0001), communication with relatives to get rid of job stress (p<.0001), exercise to get rid of job stress (p<.0001), proper sleep at night Day (p<.0001), specific illness of first-degree family members (p=.04) and shift status in emergency room (p<.0001) had a significant difference. Also, the results of the linear regression model showed that the age of doctors, the duration of marriage of doctors, the general work experience of doctors, and the experience of working in the emergency room of doctors significantly predict the score of the life expectancy test (R2=.216 and F (5,211) = 11.64 and p<.0001).
Conclusion:
Improper work shift reduces the life expectancy and then disrupts the personal, family and professional life of the treatment staff. In addition, demographic characteristics, shift work, proper night sleep, living conditions, travel, sports, relationships with relatives, specific physical or psychological illness, and specific illness of family members had a significant effect on life expectancy. Also, increasing the work shifts of doctors working in the emergency department, especially night shifts, significantly reduces the life expectancy of this group of physicians
