fa ارزیابی میزان خطاهای دستورات دارویی در نسخه‌های الکترونیک بیمه تأمین اجتماعی در یکی از داروخانه‌های مرجع شهر تهران فناوری‌های مرتبط با طب انتظامی Police Medicine Related Technologies پژوهشی اصيل Original Research <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:IRANsans;"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><strong><span lang="AR-SA">اهداف</span></strong><span lang="FA"><strong>: </strong>اشتباه در انتقال دستور دارویی پزشک به بیمار، یکی از رایج‌ترین عوامل بروز خطای نسخه‌نویسی است که می‌تواند به دلیل خط ناخوانای پزشک اتفاق بیافتد و انتظار می‌رود با ایجاد نسخه‌نویسی الکترونیک، اشتباهات ناشی از نسخه‌نویسی دستی به حداقل برسد. هدف از این مطالعه، بررسی خطاهای دستورات دارویی ثبت‌شده در نسخه‌های الکترونیک بیمه تأمین اجتماعی در سه‌ماهه ابتدای الزام به نسخه‌نویسی الکترونیک بود.</span></span></span></span><br> <span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><strong><span lang="AR-SA">مواد و روش‌ها</span></strong><span lang="FA"><strong>: </strong>این مطالعه توصیفی-مقطعی در سه‌ماهه انتهای سال 1400 در داروخانه شبانه‌روزی 13 آبان شهر تهران، انجام شد. در این مطالعه، 6210 نسخه الکترونیک دارویی در دسترس که از آبان‌ تا اسفندماه سال 1400 در سامانه نسخه الکترونیک سازمان تأمین اجتماعی ثبت شده بودند، بررسی شدند. با بررسی این نسخه‌های الکترونیک، تعداد نسخه‌های دارای دستورات دارویی غلط، دستورات دارویی مبهم و دستورات دارویی که با توضیح خاصی در نسخه الکترونیک ثبت شدند یا از واژه خاص &quot;طبق دستور&quot; در دستورات دارویی استفاده ‌کرده‌ بودند، شمارش شدند. داده‌ها با کمک نرم‌افزار&nbsp;Microsoft Excel 2010 تجزیه و تحلیل شدند.</span><span dir="LTR"><span style="font-family:"Cambria","serif""></span></span></span></span></span><br> <span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><strong><span lang="AR-SA">یافته‌ها</span></strong><span lang="FA"><strong>: </strong>از 6210 نسخه الکترونیک بررسی‌شده، 4350 (06/70 درصد) نسخه فاقد اشتباه یا ابهام در دستور دارویی بودند. 1436 نسخه (12/23 درصد) حداقل با یک اشتباه نسخه‌نویسی همراه بودند. 459 نسخه (39/7 درصد) حداقل یک ابهام در ثبت دستور دارویی داشتند. همچنین، در 3183 نسخه (26/51 درصد)، واژه &quot;طبق دستور&quot; حداقل یک بار در دستورات دارویی قید شده بود. 928 نسخه (94/14 درصد) نیز با توضیحات اضافی در دستور دارویی ثبت شده بودند.</span></span></span></span><br> <span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><strong><span lang="AR-SA">نتیجه‌گیری</span></strong><span lang="FA"><strong>: </strong>بر اساس نتایج این مطالعه، اگرچه تبدیل نسخه‌های دارویی از دست‌ نویس به الکترونیک، خطاهای مربوط به خط ناخوانای پزشک را برطرف کرده اما منشأ اشتباهات جدیدی شده است. لذا، از نظر آماری، اشتباهات در دستورات دارویی نسخ الکترونیک تغییر قابل ملاحظه‌ای نسبت به نسخه‌های دست نویس نداشته است.</span></span></span></span></span></span></div> <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:IRANsans;"><span new="" roman="" times=""><span cambria=""><span style="color:#0e101a"><strong>Aims:</strong> Mistakes in transferring a doctor&#39;s prescription to a patient are among the most common causes of prescription errors. These errors can often result from a doctor&#39;s illegible handwriting. The introduction of electronic prescriptions is expected to reduce the mistakes associated with manual prescriptions. This study aimed to examine the errors in prescriptions recorded in the Social Security insurance electronic prescription system during the first three months after the implementation of the electronic prescription requirement.</span></span></span><br> <span new="" roman="" times=""><span cambria=""><span style="color:#0e101a"><strong>Materials and Methods: </strong>This descriptive cross-sectional study was conducted in the last quarter of 2021 at the 13 Aban 24-hour pharmacy in Tehran. The research involved a review of 6,210 electronic prescriptions recorded in the Social Security Organization&#39;s electronic prescription system from November to March 2021. The study focused on identifying the number of prescriptions that contained incorrect drug entries, ambiguous drug prescriptions, and those that included specific annotations or the phrase &quot;as prescribed&quot; in the drug instructions. The data were analyzed using Microsoft Excel version 2010.</span></span></span><br> <span new="" roman="" times=""><span cambria=""><span style="color:#0e101a"><strong>Findings:</strong> In a review of 6,210 electronic prescriptions, 4,350 (70.06%) were found to have no errors or ambiguities. Conversely, 1,436 prescriptions (23.12%) contained at least one error, while 459 prescriptions (7.39%) included at least one ambiguity. Additionally, the phrase &quot;as prescribed&quot; appeared in 3,183 prescriptions (51.26%), and 928 prescriptions (14.94%) included additional explanations.</span></span></span><br> <span new="" roman="" times=""><span cambria=""><span style="color:#0e101a"><strong>Conclusion:</strong> This study&#39;s results indicate that while converting prescriptions from handwritten to electronic formats has eliminated errors associated with illegible handwriting, it has also introduced new types of errors.
